Student Registration Forms

Please bring the following information with you:

• Original birth certificate or affidavit
• Proof of residence- utility bill, lease agreement, or title
• Immunization record, including the following:
  
  • 2 doses of measles vaccine
  • 3 doses of DPT, one after 4th birthday
  • 3 doses of oral polio, one after 4th birthday
  • 2 doses of MMR
  • 3 doses of hepatitis B
  • 1 dose varicella or document child had the disease (chickenpox)
• Proof of physical examination
• Student's social security card
